Multiple peri ventricular infarcts on the top of ischemic small vessel disease (MR).








PROTOCOL:
T2W – Sagittal & axial scans
T1W – Axial scans
FLAIR – Coronal scans
DW & T2FFE (Gradient) – Axial scans
3D TOF MR angiography of the circle of Willis vessels and MIP reconstruction.

FINDINGS:

-Urgent reporting done.
-Revealed:
•    Multiple, variable sized; bilateral foci of hyper intensities in both peri ventricular and centrum semi ovale regions (better noted from image 12 to image 17 Flair sequence axial view).
•    Multiple peri ventricular restricted diffusion areas on both Diffusion and ADC maps images.
•    Suspicious small lacunar infarcts in the right side of Cervico-medullary region.
•    Mild brain and cerebellum atrophy.
•    Atherosclerotic changes on MR angiography with no focal area of stenosis or occlusion.


OPINION:

•    In light of above MR findings; picture is suggestive of acute peri ventricular area infarction on top of ischemic small vessel disease for clinical correlation.
•    Suspicious lacunar infarcts right side of Cervico-medullary junction.
