Disc herniation with caudal migration and sequestration (MR).






PROTOCOL:   T1W & T2W - Sagittal & Axial scans

FINDINGS& IMPRESSION:

-There is normal lumbar lordotic curvature.

-Modic type II degeneration of the opposing end plate of L5-S1.
-Lumbar spondylosis.
-The intervertebral disc L4-5& L5-S1 show abnormal height and abnormal hyperintense signal on T2W scans. 
L5-S1: Central disc herniation causing severe thecal indentation with caudal migration and sequestration (sequestrated piece of about 11 x 9 mm in diameters) on top of diffuse disc bulge / osteophyte complex resulting in bilateral severe foraminal encroachment.
L4-5: Central disc protrusion causing mild thecal indentation on top of diffuse disc bulge / osteophyte complex resulting in mild bilateral foraminal encroachment.  
No other significant focal protrusion or diffuse disc bulge is noted.
-Mild hypertrophy of ligamentum flava opposite L4-5& L5-S1.

-Spinal canal dimensions are adequate.
-Facet joints and the rest of the ligamentum flava show normal signal intensity.
-Thecal sac, conus and cauda equina are normal in appearance.
-Paravertebral muscles show normal MR morphology and signal intensity.
