Embolic infarction (MR).







PROTOCOL:

T1W – Sagittal & axial scans
T2W – Axial & coronal scans
FLAIR – Axial scans 

FINDINGS:

-Revealed:
Abnormal multiple variables sized altered signals in the right medial temporal lobe; right basal ganglia and right cerebellar hemisphere with T2 shine through on diffusion and pseudo normalization on ADC maps images.
Smooth halo of bilateral peri ventricular increased signal intensity. 
Another tiny area of faint hyper intensity in the right side of the pons (seen only on Diffusion images).
Brain atrophy with widened sulci and ventricular system and exaggerated basal cisterns.
Cerebellar atrophy.
Mild corpus callosal atrophy.
Non visualization of the right vertebral artery.
Non visualization of terminal branches of both middle cerebral arteries

-Brain Parenchyma shows normal grey white matter differentiation and signal intensities.  No focal mass lesion is seen.
-Fourth ventricle is normal in size and is midline in position.


OPINION:

Picture is suggestive of embolic infarction for clinical correlation.
Atherosclerosis of cerebral arteries.
